How To Fill Out the Form
As a convenience to you, the Preprofessional and Early Assurance forms have been specially formatted to accept your data when you type on the web.
- With the form displayed on your browser (see the sample image below), make sure the 'hand' tool is depressed. If it is not, click on it. Your mouse pointer will change to the 'hand'. This will permit you to type directly on the page. When you click on one of the boxes, a check mark will appear; to erase it, click on the box again.
- Fill out the form completely. (The only field you cannot fill out is the 'signature' line; sign after you print it.)
- Now simply print the page (or the entire packet if you have not done so). The form will print with all the information you entered.
- Sign the form and include it with the rest of your application.
